Summary:
The RN Case Manager's primary functions are to coordinate and assume responsibility for the coordination of patient care with the interdisciplinary team including physician, patient/family and referring agency; administer skilled nursing car for clients of all ages in their place of residence.

Graduate of an accredited Diploma, Associate or Bachelors School of Nursing.
Licenses/Certification:
Current state license as a Registered Nurse. Proof of current CPR and Hepatitis profile.
Current driver's license.
Must have reliable transportation and insurance.
Experience:
Two years experience as a Registered Nurse in a clinical setting, home health preferred.
